Activity Assistant

​

​The Activity Assistant delivers day-to-day activity support under the direction of the Activity Supervisor and Activity Lead. This includes assisting with set-up, facilitation, cueing and redirection for group activities and one-on-ones. 

​

Activity Lead

​

​The Activity Lead executes and coordinates the daily activity program in the day center, mentors the Activity Assistant, and ensures the safe, therapeutic, and engaging delivery of activities aligned to care plans created by the Activity Supervisor.

​

Activity Supervisor

 

The Activity Supervisor is responsible for conducting activity/recreation assessments for all new Participants and at required intervals. This includes development, implementation and updates to individualized Activity Care Plans.

​

Full-Time Driver
 

Responsible for the direct transportation of PACE Participants to and from their destinations, including home, the PACE Center, medical providers and more. This position requires constant interaction with Participants, caregivers, community agencies, medical providers and PACE program staff and leadership.



Scheduler

The Scheduler is responsible for scheduling internal and external appointments for PACE Participants as requested by the interdisciplinary team. This position requires direct interaction with PACE Participants, caregivers and community providers to coordinate appointments.
