Activity Assistant

​

​The Activity Assistant delivers day-to-day activity support under the direction of the Activity Supervisor and Activity Lead. This includes assisting with set-up, facilitation, cueing and redirection for group activities and one-on-ones.

​Social Work Support

​

The primary function of this position is to collaborate with Participants and/or their designated representatives to identify and obtain resources that address gaps in the care plan prepared by the Interdisciplinary Team, such as community resources to meet the social and/or emotional needs that affect the Participants’ ability to achieve their goals and assist in acquisition of social services in compliance with PACE, federal, state and local regulations.
