Full-Time Driver

The PACE driver is responsible for the direct transportation of PACE participants to and from their destinations, including home, the PACE Center, medical providers, etc. This position requires constant interaction with PACE Participants, caregivers, community agencies, medical providers and PACE program staff and leadership. This is a full-time position working Monday, Tuesday, Wednesday and Friday.

Dietitian

The Dietitian optimizes nutritional status of PACE Participants through comprehensive nutrition assessment, monitoring and communicating concerns & recommendations to the PACE Interdisciplinary Team. The Dietitian also provides comprehensive nutrition assessments and counseling to patients at the AIHS GUIDE clinic.

Personal Care Attendant & Driver

The Personal Care Attendant/ Driver provides personal care and related services in the 
PACE Center and at participant’s home. Tasks to be performed must be assigned by and performed under the supervision of the PACE Attendant Care Supervisor to ensure all 
tasks are approved in the participant’s individual care plan and performed according to 
protocol.
