Dietitian

The Dietitian optimizes nutritional status of PACE Participants through comprehensive nutrition assessment, monitoring and communicating concerns & recommendations to the PACE Interdisciplinary Team. The Dietitian also provides comprehensive nutrition assessments and counseling to patients at the AIHS GUIDE clinic.

Personal Care Attendant/Driver

The Personal Care Attendant/ Driver provides personal care and related services in the PACE Center or home, under the direction, instruction, and supervision of the Attendant Care Supervisor. Tasks include assisting with bathing, dressing, toileting, personal hygiene and maintaining a healthful, safe environment. The Personal Care Attendant/ Driver also provides direct transportation of PACE Participants to and from their destinations, including home, the PACE Center, medical providers, etc. This position requires constant interaction with PACE Participants, caregivers, community agencies, medical providers and PACE program staff and leadership.
