INTERDISCIPLINARY TEAM

The Interdisciplinary Team (IDTeam) manages and provides all PACE participant’s services. The IDTeam is made up of 11 disciplines which integrates both clinical and social care. This care is provided across all settings and even includes prescriptions, transportation & meals which is why we say At your PACE, Everything in ONE PLACE.

The IDTeam is responsible for the groundwork of a participant's care with the creation of their individualized care plan. The diversity and expertise of the IDTeam ensures that all the needs of the participant are being met with an all-inclusive, integrated, and centralized plan.
